Write down what you have noticed
- What you take and when.
- Benefits you have noticed.
- Side effects or new problems.
- Missed doses and what makes the routine difficult.
- Other medicines, supplements or substances the clinician should know about.
Questions to ask
- What is this medicine intended to help with?
- What should I do if I miss a dose?
- Which side effects need prompt medical advice?
- When should this treatment be reviewed again?
Do not make the review yourself
If you are worried about a medicine, contact the prescriber, pharmacist, GP or relevant service. Do not use a general information website as instructions to start, stop or change a prescribed medicine.
Prepare without trying to be perfect
Write down the things you most want remembered. You can read from your notes, show them to the clinician, or use the free appointment-prep tool to put them in order.
